This appears to be a traditional horn medicine bottle or horn dosing instrument, commonly used in the past to administer liquid medicine to animals like cattle. It was often crafted from animal horns and could be marked with initials or dates, as seen here. Cow horn, marked with initials "MKB" and "RB" and "RB 1778".
